Forms

To make the process easier for the participants and some of the vendors we work with, we have created many forms that automate the process:

The Medical Form, when filled out and submitted goes directly to Lifeway Medical and the office of Dr. Marah Lee, our Medical Director for the ride. No stamps, no faxes, no fuss!

The Housing Form, a great way to let our Housing Manager know what your plans are for the ride, will you be staying with us on Friday night in Hawks Cay? Will you want to have your unregistered spouse or partner with you? Maybe you want to upgrade your room, all possible on this years Housing Form, just fill it in, hit submit and BAM  your job is done.

Team Form, forming a team?, Good news our new Team Registration page allows you to put in the names of all those on your team, including their participant number, it generates an e-mail to our Registration Administrator and he sets up your team with a Team number and directs you to fill out a new team pledge form…So simple!

Transportation Form, if you don’t have a ride back from Key West on Sunday morning, we have partnered with Identity Travel for the second year to provide transportation back to MorningStar Renewal Center (our starting location). Go to the Transportation Form, fill it out and hit submit, an e-mail is sent directly to Identity Travel with your request. They will contact you to finalize your reservation…Very Easy!

Fill in and Print out your pledge form , well this one is actually a pdf file that you can add your Name and Participant # to, it’s important that all pledges arrive with their own pledge form. Why you ask? We have volunteers that put in all the checks we receive and it is very efficient if all the submissions look alike…
